1. Under the supervision of Rehab Nurse, the EP will assist in monitoring the physical and psychological well-being of
the cardiac and pulmonary rehab patient
a. Checks supplies and performs daily code cart per policy and records results
b. Able to operate exercise equipment
c. Able to recognize abnormal heart rhythm and initiate proper emergency response
d. Conducts group exercise programs as assigned
e. Able to assess accurate VS including BP, HR, Resp and O2 saturation.
f. Communicates with RN regarding patient's condition, reaction and responses to exercise.
2. Collaborates with the rehab team to develop and monitor the individualized treatment plan. Implements the
treatment and interventions and will continuously evaluate and document the patient's progress towards attainment
of goals and expected outcomes.
a. Collaborates with RN and PT to develop individualized exercise prescription and goals
b. Documents patient assessment, plan interventions and response in accordance with applicable policies and
procedures
c. Maintains medical record including assessment, interventions and clinical response
d. Performs Reassessment and goal adjustment according to policy
e. Documents patient outcomes that reflect goal achievement.
3. Provide quality care based on patient needs, established standards of care and according to policy and procedures
a. Demonstrates the ability to provide competent, effective care.
b. Demonstrates the ability to use all applicable equipment and software applications
c. Provides care that maintains patient confidentially, comfort and safety
d. Prioritizes and organizes work appropriately to effectively meet patient needs
e. Provides care in accordance with hospital policies/procedures and dept. protocols
f. Collaborates and communicates relevant patient data to appropriate persons clearly and effectively
4. Demonstrates knowledge of unit / department quality improvement program or process.
a. Demonstrates contribution to unit based programs and strategies to monitor continuously improve the
quality of patient care.
b. Identifies opportunities for improvement in processes of care and other improvements at dept. level.
c. Actively participates in the collection and use of date and or evaluation relative to unit based improvements
5. Demonstrates clinical skills necessary for pertinent and accurate data collection.
a. Demonstrates competency in the performance of duties and responsibilities related to care of the rehab
population
b. Identifies education needs and seeks opportunities for improvement
c. Completes annual rehab competencies
d. Shows commitment to learning new knowledge and skills to enhance services to patients
Qualifications
Experience Minimum Required
Work or internship experience in phase 2 and 3 cardiac and pulmonary rehab. Experience in assessment, goal
development and interventions for the cardio/pulmonary patient population.
Successful completion of related internship
Experience Preferred/Desired
1 year experience in phase 2 and 3 Cardiac and pulmonary Rehab
Education Minimum Required
Bachelors degree in Exercise Physiology
Education Preferred/Desired
Training Minimum Required
Training in basic EKG recognition, exercise prescription and evaluation of goals and outcomes for the rehab
patient.
Training Preferred/Desired
Training in Cardiac and pulmonary rehab or equivalent combination of education and experience
Special Skills Minimum Required
Professional in interacting with people of various backgrounds, skill levels, and value systems. To have a calm,
reassuring demeanor. To have the ability to provide excellent internal and external customer service and must
also have the ability to multi-task and prioritize.
Licensure and/or Certifications Required
BLS for Healthcare Provider (upon hire) Basic Arrhythmia (within 6 months of hire) Advanced Cardiac Life
Support(within 1 year of hire)
Licensure and/or Certifications Preferred/Desired
